Living Healthy Program (The Chronic Disease Self-Management Program)
Working with the North Carolina Division of Aging and Adult Services (DAAS) and Community Care of Wake and Johnston Counties, the Diabetes Prevention & Control Program conducts workshops and workshop leader trainings to persons with chronic illnesses and health care professionals to reduce the risk and occurrence of disease complications and disability. Outreach has focused on adults 60 and older, minority populations, and Medicaid recipients. Workshops are taught using the Chronic Disease Self-Management Program (CDSMP) curriculum, an evidence-based program developed by researchers at Stanford University. CDSMP is a highly participatory workshop that takes place once a week for six weeks. Each 2 ½ hour session of the workshop is facilitated by 2 trained leaders, who follow a detailed manual, so that each workshop is highly consistent. Subjects covered include: (1) dealing with frustration, fatigue, pain and isolation, (2) appropriate use of medications, (3) communicating effectively with friends, family, and health care providers, (4) appropriate exercise, (5) healthy eating, and (6) making informed treatment decisions. CDSMP does not replace disease-specific education, rather it enhances regular treatment. |
