Purpose
The overall purpose of the North Carolina Statewide Diabetes Plan is to:
- Decrease complications including amputations, blindness, end stage renal disease and cardiovascular disease for those with diabetes,
- Decrease the incidence and delay the onset of diabetes among populations who are at risk.
Outcomes
Four measurable outcomes will be the direct result of the implementation of this plan. These outcomes address specific issues relevant to diabetes in North Carolina and are responsive to national standards set forth by the Centers for Disease Control and Prevention. Both statewide and program-specific data will be used to track accomplishments of these outcomes. The outcomes are:
- By 2008, increase the percentage of persons with diabetes in NC who received the recommended foot and eye exams, influenza and pneumococcal vaccines, and test for hemoglobin A1c, lipids and microalbumin. The baselines for each of these indicators were recalculated in 2003, and are available upon request.
- By 2008 and upon completion of the primary prevention assessment process, plan a diabetes prevention program designed to: A) inform, educate and empower people at risk for type 2 diabetes to be tested; B) inform, educate and empower health care providers in advising people at risk to be tested and to obtain healthy weight; C) increase the number of at risk people who are tested in the clinical setting; and D) decrease the risk factors for type 2 by fostering policies and environments supportive of increased physical activity and healthy eating initiatives.
- By 2008, through collaboration and linkages with state and local partners, foster policies and environments supportive of healthy eating and increased physical activity and promote preventive health practices for people with diabetes and those at-risk. This will be evidenced by data collected from interventions including Diabetes Today, DIRECT, and NC BASIC.
- By 2008, reduce health disparities for high-risk populations with respect to diabetes prevention and control.
